Objective: This study tried to investigate the effects of the overdose of the tramadol, and included the assessment of the toxicity of serotonin based on Hunter Serotonin's Toxicity Criteria, where the seven clinical characteristics of the spontaneous clone, the inducible clone, the eye clone, agitation, diphoresis, tremor and central hyperreflexia are examined in all patients who take. Materials and methods: This is a series of observation cases based on a retrospective review of tramadol overdose (Council 400 mg) admitted to a tertiary toxicology unit from November 2000 to June 2013. Demographic data, ingestion information (dose and co-ingestants), clinical effects, complications (left, serotonin toxicity and cardiovascular effects) and the admission of the intensive care unit (ICU) were extracted from a clinical database. Results: There were 71 cases of tramadol overdose (average age: 41 years, range: 17-69 years; and ingested average dose: 1000 mg, interquartile range [IQR]: 800-2000 mg). Seizures were associated with doses and occurred in 8 patients, one of them co-induced a benzodiazepine compared to 16 patients without seizures. There were no cases of serotonin toxicity knowing the Hunter Serotonin Toxicity Criteria. Tachycardia occurred in 27 and mild hypertension occurred in 32. The Glasgow Coma Score was performed 15 in 29 and was performed 9 in 5 patients; three co-induced tricyclic antidepressants and two tramadol alone (3000 mg and 900 mg). Respiratory depression occurred in 13, mean doses: 2500 (IQR: 1600-3000) mg which was significantly different (p = 0.003) to patients without respiratory depression, medium doses: 1000 (IQR: 750-1475) mg. Eight patients were admitted to ICU, five for coingtant toxicity and three for respiratory depression. Discussion: Tramadol's overdose was associated with a significant risk of convulsions and respiratory depression in more serious cases, both related to the ingested dose. There were no cases of serotonin toxicity, while the effects similar to opioids and adrenergic effects were prominent. Conclusion: Tramadol overdose is associated with convulsions and respiratory depression, but it is unlikely to cause serotonin toxicity.
